What is my login ID?

The login ID is unique to employee and location. Your login is your facility ID plus employee ID number. Please see the chart below for your facility code. If you are unsure of your employee ID number used for ordering on the uniform website, contact your HR department. An example login ID for a LaPorte employee with employee ID # 12345 is LS12345.
